The doctor said to the patient, “Open your mouth”.
The doctor told the patient to open his mouth.
To change from direct to indirect follow these steps:
1. Said to  changes to told
2. Omit the brackets “ ’’ and use to. Bring the verb in the infinitive after to. If the order is in negative we put not to + infinitive.
3. Change the pronouns according to the speaker.
 The traffic policeman said to the driver," Pay the fire”.
The traffic policeman ordered the driver to pay the fire.

 Mr. Ali said to the children, “Don’ts play in the street”.
Mr. Ali warned the children not to play in the street.

 My friend said," You should see a doctor soon”.
My friend advised me to see a doctor soon.
Note:
1. Sometimes (said to in orders) changes to the following words but it depends on the meaning only. (Told – ordered – advised – warned – asked – begged).
